This form is required for requesting access to Banner Admin Pages (System Screens), UC4 (Appworx), Argos, Workflow, BDM, and Cloudshare. The first page, including all required signatures, must be completed. A Jira ticket must be created for the Security Request, and all access forms must be attached to the ticket. Each data owner, as identified at the bottom of the respective form, must be added as a watcher on the ticket. Data owners are responsible for reviewing, signing, and uploading the signed request forms to the Jira ticket. Access will be granted only after all required signed forms have been uploaded and verified.
